Note to Parents
Welcome to the
Champaign Unit 4 Schools student registration web site.
Please read the following information
before starting the on-line registration process.

In accordance with the policy of the Champaign
Unit 4 Schools, students who are five years old on or before September 1st
of their Kindergarten year may attend school.
·
Parents must come to the
Family
Information
Center
to complete their student’s registration process.
·
Parents must bring proof of
Champaign
residence (i.e., Drivers License, Mortgage, Lease, Voter’s Registration Card,
Medical Card, and Contract to build.)
·
Mail or bills cannot be used as proof of
residence.
·
Parents must also provide the student’s Certified
Birth Certificate, Physical and Immunization Records, Vision Exam and Dental
Exam. Medical records are due
prior to the first day of school, and the Certified Birth Certificate is due
within thirty (30) days of enrollment
